We, the undersigned natural persons of the age of twenty-one years or more, acting as incorporators of a corporation under the NON-PROFIT CORPORATION ACT (D.C. Code, 1981 edition, Title 29, Chapter 5), adopt the following Articles of Incorporation:
FIRST: The name of the corporation is (District of Columbia NAME OF CORPORATION).
SECOND: The period of its duration is (PERPETUAL OR A SPECIFIED PERIOD).
THIRD: The purpose or purposes for which the corporation is organized are: (District of Columbia SPECIFIC PURPOSES OF CORPORATION: GENERAL PURPOSES ARE NOT ACCEPTABLE).
FOURTH: State whether the corporation shall have members.
FIFTH: The corporation is to be divided into (District of Columbia NUMBER) classes of members. State the designation of each class of members, the qualifications and rights of the members of each class, including the right to vote if pertinent.
SIXTH: A statement as to the manner in which directors shall be elected or appointed, if the directors or any of them are not to be elected or appointed by one or more classes of members, or that the manner of such election or appointment of such directors shall be provided in the bylaws.
SEVENTH: Provisions for the regulation of the internal affairs of the corporation, including provision for the distribution of assets on dissolution or final liquidation.
EIGHTH: The address, including street and number of the initial registered office of the corporation is (District of Columbia ADDRESS - POST OFFICE BOXES ARE NOT ACCEPTABLE) and the name of the initial registered agent at such address is (District of Columbia AGENT'S NAME - IF AN INDIVIDUAL, MUST BE A D.C. RESIDENT).
NINTH: The number of directors constituting the board of directors is (MINIMUM OF THREE REQUIRED), and the names and addresses, including street and number and zip code of the persons who are to serve as directors until the first annual meeting or until their successors are elected and shall qualify are:
